Advance booking required. Voucher must be booked by Fri 25 Nov 2016 (we don’t want you to miss out!).

Groups cannot be booked and receive treatment at the same time.

Treatment is not suitable during pregnancy/breastfeeding.

Treatment is not suitable for cancer sufferers/those in remission.

Treatment duration:  approx. 1hr 20mins in total. Visit 1: approx 20mins, inc health check, impressions of top & bottom teeth and cleaning. Visit 2: approx 1 hour, inc treatment – whitening gel applied to the tooth surface and an activating lamp is used to speed up the gel.

Teeth whitening procedure can only be carried out on natural teeth and will not work on dentures, veneers, crowns, bridges, braces or fillings.

Treament may not be carried out if the patient does not pass the dental health check/suitability tests/if they have hypersensitive teeth. Please note if patient does not qualify for procedure after consultation period The Berkeley Clinic will refer patient back to itison with refund reference code to receive a refund for their voucher.

Some members may experience short term sensitivity.

Treatment lasts up to one year, dependent on patient.

Patients may also require remedial dental treatment before whitening, this will be charged at standard rates or you can visit your own dentist for treatment and once complete return to the clinic for whitening.

GDC dentist will carry out the examination and prescribing; GDC dentist or GDC hygiene therapist will carry out whitening.

Treatments cannot be substituted for other treatments.

The beginning and end period of a voucher’s validity is always the busiest. We advise members to book well in advance to avoid disappointment.

Vouchers must be printed for redemption; please print your voucher and bring along with you to avoid disappointment.

Voucher is not exchangeable for cash or valid with any other offers.
