• Voucher is valid from Wed 4 Jan 2017 until Fri 31 Mar 2017, Mon, Wed, Fri, 9am – 5.30pm, Tue & Thur 9am - 7pm and Sat 10am – 5.30pm; subject to availability.
• Excludes Sunday.
• Advance booking required. Voucher must be booked by Wed 15 Mar 2017 (we don’t want you to miss out!)
• You risk forfeiting your voucher if you cancel within 48 hours of your booking.
• Max of 1 voucher per person and 1 voucher can be purchased as gifts.
• Over 18’s only.
• Treatment is not suitable during pregnancy/breastfeeding.
• Please discuss any health concerns directly with clinic.
• Treatment duration: 2 hours
• Treatment lasts approx. 18 months depending on patient’s diet and lifestyle.
• Patients should avoid drinking tea coffee carbonated soft drinks or food stuffs with high colouring immediately after the procedure.
• The procedure is suitable for patients with fillings crowns bridges and veeners and partial dentures; existing restorations will not change with whitening and they may require replacement restorations to achieve a balanced result.
• Treatment is not suitable for patients with fixed braces.
• Whitening treatment results are expected to be up to 6 shades lighter.
• If not suited to treatment upon consultation a full refund will be given.
• 6% hydrogen peroxide chairside light activated whitening gel.
• Most common side effect is tooth and gum sensitivity – this product contains 2 mild desensitising agents.
• Risks are possible short term sensitivity during and following procedure for around 24 hours post procedure
• If the patient wishes to maintain the appearance of white teeth they would need to top up the treatment.
• All information including risks and benefits will be provided in the consent form.
• The beginning and end period of a voucher’s validity is always the busiest. We advise members to book well in advance to avoid disappointment.
• Vouchers must be printed for redemption; please print your voucher and bring along with you to avoid disappointment.
• Voucher is not exchangeable for cash or valid with any other offers.
