• Voucher is valid until Fri 30 Mar 2018, Mon - Fri 9am - 5.30pm; subject to availability.
• Excludes Saturday and Sunday.
• Advance booking required. Voucher must be booked by Wed 28 Feb 2018 (we don’t want you to miss out!).
• You risk forfeiting your voucher if you cancel within 24 hours of your booking.
• Multiple voucher purchases permitted.
• Over 18’s only.
• Groups cannot be booked and receive treatment at the same time.
• Option 1 is suitable during pregnancy/breastfeeding; option 2 is not suitable during pregnancy/breastfeeding; option 3+4 are suitable if breastfeeding but not during pregnancy.
• Treatment may suitable for cancer sufferers/those in remission; please discuss directly with the clinic.
• Please discuss any health concerns directly with clinic.
• Patients must be dentally fit to have the treatment, if the examination showed up decay in teeth or gum disease this would need to be addressed before treatment could be carried out.
• Please note tooth whitening will only work on natural teeth and will not work on dentures, veneers, crowns, bridges or fillings.
• Quick Straight Teeth is suitable for mild/moderate crowding. If more severe crowding or significant corrections in overbite/overjet are required it may not be suitable.
• Option 1 duration: approx. 30mins.
• Option 2 includes consultation and impressions for tooth whitening trays, return appointment for scaling and tray fit; tooth whitening trays are worn for 14 days.
• Option 3 and 4 includes initial examination with x-rays, if suitable for treatment then impressions would be taken, return appointment 2 weeks later for ceramic braces fitting and additional monthly appointments for adjustments. Retainers and tooth whitening would then be provided. Treatment time is approx. 6months but can be from 3 - 12 months.
• Treatments cannot be substituted for other treatments.
• No wheelchair access available.
• The venue may restrict the number of itison bookings on each day so please book early if you want a specific date.
• The beginning and end period of a voucher’s validity is always the busiest. We advise members to book well in advance to avoid disappointment. 
• Vouchers must be printed for redemption; please print your voucher and bring along with you to avoid disappointment. 
• Voucher is not exchangeable for cash or valid with any other offers.
