Small print

• Voucher is valid until 21st August 2013, Mon-Fri 9am-5pm; subject to availability.
• One voucher can be redeemed per person; one additional voucher can be purchased as a gift.
• Appointments must be booked by 20th June 2013 or you risk forfeiting your voucher.
• Voucher includes 3 appointments; consultation, follow up & treatment itself will take 30mins.
• Consultations need to be taken by 1st July, all treatments & follow up appointments must be completed before expiry date.
• Over 18s only.
• Not suitable if pregnant, suffering from cancer or in remission. Also not suitable for those with false teeth, dentures of braces.
• Members with crowns & veneers are advised to contact Dental Care Perth prior to appointment to discuss suitability.
• Group bookings are not permitted.
• Treatment can last up to 5 months dependent on diet & care of individual.
• Advanced booking required. Members must book 4 weeks in advance of their preferred date. The beginning and end period of a voucher’s validity is always the busiest. We advise members to book well in advance to avoid disappointment.
• You risk forfeiting your voucher if you cancel within 48 hours of your appointment.
• Vouchers must be surrendered for redemption; please print your voucher and bring along with you to avoid disappointment.
• Voucher is not exchangeable for cash or valid with any other offers.
